Here is an example of creating an online check in for adaptable for not only your students but also parents and staff.
I made my school counselor online check in form to be proactive when responding to recent global events. It was an obvious action I needed and wanted to take in order to transition my school counseling program virtually where all of my students would be starting school for the upcoming 2020-2021 school year. Please, for all my fellow school counselors and even school counseling interns, create an online check in form!

The information contained in the form is short and sweet and it is the same information used on my paper copy (on the right) that I would give out to teachers in the beginning of the school year that students can request in class, fill out, and send to me when they need my help during class time.
For creating the check in form, I used both Google Forms and Microsoft Forms to prepare for which ever platform my school would transfer our online learning structure to. For both platforms, it was fairly simple to create. With Google Forms, I used canva.com to create my banner which then allowed me to choose the background color for the page. Here is my Google Form below:

With Microsoft Forms, I used the same prompts as my Google Forms but I had to play around with the background design for it to show what I wanted to display.

Overall, designing my Google form and Microsoft form respectively was a fairly simple process that took minimal time from start to finish. There are so many examples online on Pinterest and Instagram. Check them out!
